Absence Request Form

This form is for current members who need to request an absence from a band-related activity.  Please specifiy the event and date in the subject line and provide detailed information about the absence in the message box.  This form does not need to be filled out for an absence from band class.  Submitting this form does not replace calling the school attendance secretary and will not count as an excused absence from school; it is only for band events.  Your message will be sent directly to the band office.  Please submit your request at least one week in advance.*  Mr. Sanchez will review requests and follow-up with students after receiving a request.

*Emergencies are dealt with on a case-by-case basis.  If you are able, please fill out the form even when your absence is last minute/unforseen.  Our staff desires to be aware of your situation and know all our members are safe and accounted for.
